冲洗液温度对前列腺等离子电切术患者围术期体温及血液流变学的影响

Effect of irrigation fluid temperature on patients’ body temperature and hemorheology during perioperative period of PKRP

[摘要] 目的 比较经尿道前列腺等离子电切术(PKRP)采取不同温度冲洗液对患者体温及血液流变学的影响。方法 选择60例拟行PKRP的老年患者,ASAI~III级, 选择L2~3间隙行腰-硬椎管内联合麻醉。冲洗液为0.9%氯化钠溶液。将患者随机分为两组,每组30例。I组为室温冲洗液;II组为加温至37oC冲洗液。观察记录不同时间患者直肠温度、血压、心率,于术前(T0)、术毕(T1)及术后6h(T2)、术后24h(T3)抽取静脉血测定血红蛋白(Hb)和红细胞压积(Hct); 血浆电解质、血液流变学各指标。结果 室温组手术后体温明显低于手术前(P<0.05);加温组手术后体温无明显变化。两组电解质无明显变化。细胞压积均有不同程度的变化。全血粘度及还原粘度两组术中均有降低,但术后24小时室温组明显增高(P<0.05)。 结论 PKRP期间灌洗液温度能引起体温改变和血液稀释,低体温时可致术后血液粘滞度增加。

Abstract:

[Abstract] Objective: To compare the effect of irrigation fluid temperature on patients’ body temperature and hemorheology during plasmakinetic resection of prostate (PKRP) by urethral. Methods: 60 elderly patients scheduled for PKRP were selected for the study, ASAI-III. L2-3 combined spinal epidural anaesthesia was applied. The 0.9% sodium chloride solution was chosen as the irrigation fluid. The patients were randomly divided into two groups using different temperature and volume of irrigation fluid. There are 30 patients in each group .. group I use the irrigation fluid of room temperature; while group II use the fluid of 37oC. Observating the patients’ rectal temperature, blood temperature, heart rate, and measuring hemoglobin (Hb) , hematocrit (Hct) , the indicators of plasma electrolytes and hemorheology by extracting venous blood before operation (T0), at the end of operation (T1), 6 hours after operation (T2) and 24 hours after operation (T3). Results: The temperature of the patients after operation in group I was significantly lower than that before surgery (P<0.05), while group II has no significant changes. There is on significant changes of electrolytes in both of the two groups. Hematocrit changes in different degrees. Whole blood viscosity and viscosity were reduced in both groups,, but groupI was significantly 24hours after operation. Conclusion:The temperature of irrigation fluid during PKRP can cause the changes of patiengts’body temperature and hemodilution. hypothermia can increase blood viscosity after surgery.
