异氟醚预处理在脑缺血半暗带保护中的作用机制研究进展

摘要:

背景 缺血半暗带为脑缺血疾病临床治疗中的主要靶点,然而有效安全的非溶栓防治措施多年来一直未有突破。近年来有研究发现吸入麻醉药异氟醚预处理对脑缺血半暗带有保护作用,为脑缺血的防治提供了新的思路。目的 分析总结异氟醚预处理在脑缺血半暗带保护中的作用及其可能的机制。内容 首先介绍缺血半暗带在脑缺血疾病中的地位及复杂性;然后描述异氟醚预处理对脑缺血半暗带保护作用的现状并分析其可能参与的机制。 趋向 Toll样受体4信号通路的参与及其他保护机制还有待进一步探索,为其临床应用奠定更有力的证据,同时还有利于从机制中挖掘治疗脑缺血疾病的新方向。

Abstract:

Background Penumbra is the major target for cerebral ischemia treatment, but effective and secure strategies of non- recanalization therapeutic have been without breakthrough for many years. However the recent discovery that isoflurane preconditioning can protect penumbra provides new ideas of prevention on cerebral ischemia. Objective To sum up the effect and mechanisms of isoflurane preconditioning-induced cerebral penumbral protection. Content First the position and complicacy of penumbra in ischemic diseases have been introduced; then the current situation of effect and the possible mechanisms of isoflurane preconditioning-induced cerebral penumbral protection has been described and analyzed respectively. Trend The involvement of toll like receptor 4 signaling pathway and other protective mechanisms still need further exploration, which can both provide more powerful evidences for its clinical application and contribute to excavate new direction for ischemic treatment.
