体重指数对全麻患者罗库溴铵肌松作用的影响

The influence of BMI on neuromuscular blocking of rocuronium in patients under general anesthesia

【摘要】 目的 研究体重指数（body mass index，BMI)对患者全身麻醉时罗库溴铵肌松作用的影响。 方法 择期行全麻手术患者80例，ASA分级Ⅰ或Ⅱ级，80例，年龄20～60岁。根据BMI将患者分为3组：超重组（Ⅰ组，n=30），正常体重组（Ⅱ组，n=30）和低体重组（Ⅲ组，n=20）。5s内推注罗库溴铵0.9mg/kg，记录从推药到T1为0%的时间（起效时间）、无反应期(T1为0%的时间)和恢复指数（T1由25%至75%的时间）。 结果 Ⅰ组、Ⅱ组和Ⅲ组的起效时间分别为（75±15）s、（91±23 ）s和（135±27）s，Ⅰ组的起效时间短于Ⅱ组和Ⅲ组 (P<0.01)，Ⅱ组又短于Ⅲ组 (P<0.01)。Ⅰ组、Ⅱ组和Ⅲ组的无反应期分别为（49±14）min、（37±8）min和（28±7）min，Ⅰ组的无反应期则长于Ⅱ组和Ⅲ组 (P<0.01)，Ⅱ组又长于Ⅲ组（P<0.01)。三组的恢复指数的比较差异无统计学意义(P>0.05)。 结论 按体重静注罗库溴铵， BMI是影响该药起效时间和无反应期的重要因素之一。

Abstract:

【Abstract】 Objective To investigate the influence of Body Mass Index （BMI）on neuromuscular blocking of rocuronium in patients under general anesthesia. Methods Eighty ASA Ⅰ or Ⅱ patients，aged 20-60 yr，undergoing elective surgery under general anesthesia，were divided into 3 groups：overweight (groupⅠ) group，nomal body weigh (groupⅡ) group and underweight (group Ⅲ) group.To give rocuronium 0.9mg/kg in 5s，the time from the start of injection the drug to T1 was 0%(onset time) and the nonresponse period were recorded，the recovery index was recorded too. Results Onset time in groupⅠ、Ⅱ and Ⅲ were （75±15）s、（91±23）s and （135±27）s respectively.Onset time in groupⅠwas shorter than that in group Ⅱ and group Ⅲ(P<0.01) ，and that in groupⅡ was shorter than in group Ⅲ(P<0.01) .The nonresponse period in groupⅠ、Ⅱ and Ⅲ were （49±14）min、（37±8）min and （28±7）min respectively.The nonresponse period in groupⅠwas longer than that in group Ⅱ and group Ⅲ(P<0.01) ，and that in groupⅡ was longer than in group Ⅲ(P<0.01).The recovery index have not significant difference in three groups(P>0.05) . Conclusion To give rocuronium according to body weight，BMI is an significance factor to affect the onset time and nonresponse period of rocuronium.
