舒芬太尼预处理对浸水束缚应激糖尿病大鼠应激性胃黏膜损伤的影响

摘要:

目的 观察舒芬太尼预处理对浸水束缚应激(WIRS)糖尿病大鼠急性胃黏膜损伤的影响及可能保护机制。方法 急性胃粘膜损害模型采用经典WIRS改良法复制。雄性成年SD大鼠50只,随机分成五组,每组10只:正常对照组(A组),正常大鼠WIRS组(B1组),糖尿病大鼠WIRS组(B2组),舒芬太尼预处理+B1组(C1组),舒芬太尼预处理+B2组(C2组)。WIRS大鼠6h后处死取胃组织标本,显微镜下观察胃黏膜损伤指数(GI)与组织学改变,比色法测定各组胃组织中MDA、SOD、MPO含量。结果 WIRS6h后,各组大鼠均发生不同程度的急性胃粘膜损伤,与A组差别有非常显著统计学意义,其中B组最为严重,且B2组GI又明显高于B1组(P0.05);舒芬太尼预处理可有效减轻WIRS正常大鼠和糖尿病大鼠AGML的损伤,但与A组比较差别仍有显著统计学意义(P0.05),其中C1和C2两组间比较差别无统计学意义。光镜下见大鼠WIRS6h后胃粘膜均出现不同程度形态学改变,其损伤程度以B2组最为严重,经舒芬太尼预处理后充血、水肿减轻,炎细胞浸润明显减少。结论 与正常大鼠相比,糖尿病大鼠更容易发生并加重WIRS所致急性胃粘膜损伤。舒芬太尼预处理可以减轻这种WIRS所致的急性胃粘膜损伤,其机制可能舒芬太尼对伤害性应激反应的控制,以及氧自由基清除增加和脂质氧化减少有关。

Abstract:

Objective To observe the protective role of sulfentanyl pretreatment on acute gastric mucosal lesion induced by water immersion restraint stress(WIRS) in diabetic rats and investigate its’ possible mechanism. Methods Eighty adult SD rats were randomly divided into eight groups(n=8, each), including normal rats (Group A), normal rats suffering WIRS(group B1), diabetic rats suffering WIRSs(Group B2), normal rats suffering WIRS pretreated with sufentanil(Group C1), diabetic rats suffering WIRS pretreated with sufentanil(Group C2), normal rats suffering WIRS were pretreated with sufentanil plus specificity. Group B2, C2 are all diabetic rats. The acute gastric mucosal lesion models were copied by the classic WIRS,. All the experimental rates will killed at 6 hours after WIRS for stomach samples including the namoral rats in group A. Gastric mucosal injury index(GI) was assessed thought 10x dissecting microscope and pathologic histology were observed under optical microscope of each group. MDA, SOD and MPO of gastric tissue homogenate were measured with chromatometry. Results All rats who suffer WIRS can be found acute gastric mucosal lesion, GI is significantly higher in Group B2 vs Group B1 and C1 (p<0.05); GI is significantly higher in Group D2 and D3 vs Group C2(p<0.05), while there is no obviously differ between Group D1 and Group C2 (p>0.05). It can be found diversity degree of engorgement hydropsia and inflammatory cell infiltrate in those rats’ gastric mucosa at 6h after WIRS, those symptom is wost in group B2 and better in groups which were pretreatment with sufentanil. Conclusions It seams that diabetic rats will get severer injure of stomach mucous membrane than normal rats. Pretreatment with sufentanil can effectively protect stomach mucous membrane from WIRS-induced injury, the depression of lipid peroxidation, released oxygen radicals and increased neutrophils.
