Azasetron versus azasetron in combination with dexamethasone for the prophylaxis of postoperative nausea and vomiting in patients undergoing laparoscopic surgery: a meta-analysis of randomized controlled trials

摘要 目的 评价阿扎司琼单独或联合应用地塞米松降低腹腔镜术后恶心呕吐的效果。 方法 检索Pubmed,Embased, Medline, Cochrane Library,中国学术期刊全文数据库(CNKI),中国生物医学文献数据库(CBM)、万方数据库中阿扎司琼联合地塞米松与阿扎司琼单独应用在全麻腹腔镜手术中的对比应用,利用软件Revman 5.3进行系统评价。 结果 7项研究纳入Meta分析,其中阿扎司琼联合地塞米松组275例,阿扎司琼组275例。相比阿扎司琼,阿扎司琼联合地塞米松能明显降低腹腔镜术后恶心呕吐(PONV)发生率,恶心、呕吐、总的PONV合并效应、腹腔镜胆囊手术的PONV合并效应及腹腔镜妇科手术PONV合并效应的危险比(RR)与95%可信区间(95%CI)分别为0.64[0.41,0.99]、0.27[0.15,0.49]、0.49[0.14,0.69]、 0.48[0.29,0.77]、0.50[0.34,0.84]。 结论 阿扎司琼联合地塞米松相比单独应用阿扎司琼可明显降低腹腔镜术后PONV发生率。

Abstract:

【Abstract】Objective To evaluate the effect of azasetron versus azasetron in combination with dexamethasone in patients undergoing laparoscopic surgery. Methods We search the Pubmed, Embased, Medline, Cochrane Library, CNKI, CBM, Wanfang Data Base to identify randomized controlled trials (RCTs) about compared azasetron with azasetron in combination with dexamethasone in patients undergoing laparoscopic surgery.The meta-analysis were performed by Revman 5.3 software. Results A total of 7 RCTs , including 275 cases receiving azasetron and 275 cases receiving azasetron in combination with dexamethasone were eligible for meta-analysis. The preventive efficacy of azasetron in combination with dexamethasone against PONV was better than azasetron group. The RR of nausea, vomiting, total PONV,PONV in laparoscopic cholecystectomy, PONV in gynecological laparoscopic surgery and 95%CI were 0.64[0.41,0.99], 0.27[0.15,0.49], 0.49[0.14,0.69], 0.48[0.29,0.77], 0.50[0.34,0.84], respectively. Conclusion azasetron in combination with dexamethasone can significantly decrease the incidence of PONV compared with azasetron itself.
