不同年龄段婴幼儿静吸复合麻醉中应用顺阿曲库铵的肌松时效研究

目的:研究不同年龄段婴幼儿静吸复合麻醉中应用顺阿曲库铵的肌松时效,探讨顺阿曲库铵的合理使用剂量。方法:以2015年10月~2016年11月在我院采用静吸复合麻醉进行手术的100例婴幼儿为研究对象,根据年龄段将婴幼儿分为两组,A组(3~6个月),B组(7~36个月),再根据顺阿曲库铵负荷剂量(2ED95 0.10mg/kg, 3ED95 0.15mg/kg)将婴幼儿平均分为两个亚组,共A0.10、A0.15、B0.10、B0.15四组,每组25例。观察肌松时效指标(起效时间、临床作用时间、RI、Th时间与TOF恢复时间)与不良反应情况。结果:A组的起效时间显著短于B组(P<0.05),高负荷剂量组(0.15mg/kg)的起效时间显著短于低负荷剂量组(0.10mg/kg) (P<0.05);临床作用时间与恢复时间在各组间的比较差异无统计学意义(P>0.05);各组间不良反应情况比较差异无统计学意义(P>0.05)。结论:婴幼儿的年龄与顺阿曲库铵的负荷剂量是影响肌松时效的关键因素。

Abstract:

OBJECTIVE To study the muscular relaxat effect of cisatracurium in infants and young children with different dose under intravenous inhalational anesthesia, in order to discuss the reasonable using dosage. METHODS 168 cases of infants and young children were research object which was carried out surgery under Intravenous inhalational anesthesia in our hospital during 2015.10~2016.11. 168 cases of infants and young children were divided into group A(3~6 months)and group B(7~36 months) by age-group. Group A and group B was divided into two subgroups by loading doses of cisatracurium (2ED95 0.10mg/kg, 3ED95 0.15mg/kg). All four groups: group A0.10, group A0.15, group B0.10 and group B0.15, 25 cases per group. The muscular relaxat effect index (onset time, clinical action time, RI, Th time and TOF recovery time) and untoward effect was observed. RESULTS The onset time of group A was significantly shorter than group B(P<0.05) and the onset time of high loading doses group (0.15mg/kg) was significantly shorter than low loading doses group (0.10mg/kg) (P<0.05). The difference of clinical action time, recovery time and untoward effect had no statistical significance between four groups(P>0.05). CONCLUSION The age of infants and young children and loading doses of cisatracurium was the key factor of influencing muscular relaxat effect.
