基于术后疼痛管理指南的患者自控静脉镇痛护理管理方案构建与应用

摘要:

目的 构建基于术后疼痛管理指南的患者自控静脉镇痛(patient-controlled intravenous analgesia, PCIA)护理管理方案(以下简称“方案”)并探讨其应用效果。 方法 基于2016年美国疼痛学会发布的术后疼痛管理指南,拟定腹部手术后方案初稿,采用专家会议法确定终稿并组织实施。比较方案实施前后,两组患者对镇痛服务的满意度、PCIA撤除原因、PCIA不良反应发生数、患者对不良反应处理的满意度、反馈的术后疼痛程度及其对疼痛控制不足处理的满意度。 结果 方案实施后的351例患者与实施前的350例相比,患者对镇痛服务的满意度明显提高,在PCIA泵药液使用完毕前患者随意停用或撤泵的人数明显减少,主诉术后疼痛控制不足的患者比例明显增加,且差异均有统计学意义(P<0.05);两组患者在PCIA不良反应发生例数、患者对发生不良反应处理以及对术后疼痛控制不足的处理满意度比较,差异无统计学意义(P>0.05)。 结论 基于术后疼痛管理指南,构建的适于常态化运行的以护士为主体的方案能够真正发挥护士在术后疼痛管理中的地位与作用,提高术后疼痛管理的质量和患者满意度,为患者提供安全而有效的镇痛服务。

Abstract:

Objective To construct the patient-controlled intravenous analgesia (PCIA) nursing management protocol based on guideline and to explore its effect. Methods According to the guideline on postoperative pain management which was issued by the American Pain Society in 2016, the first draft of the PCIA nursing management protocol was constructed. Subsequently, the final draft of the management protocol was determined and implemented after experts′ consultation meeting. Patient′s satisfaction rates of analgesic service (PCIA), reasons for removal of PCIA, adverse effect rates of PCIA, patients′ satisfaction extent with adverse reaction management, the extent of complaining of pain control, and patients′ satisfaction rates with treatment of inadequate pain control were compared between two groups of patients before and after intervention. Results Comparing with the control group (350 patients), the patient′s satisfaction rate in experimental group (351 patients) with analgesic service was significantly improved. The number of patients with abnormal reasons for the early removal of PCIA was significantly reduced, and the proportion of patients complaining inadequate control of pain was increased significantly (P<0.05). There was no statistically significant difference between the two groups of patients in the incidence of adverse reactions of PCIA and patients′ satisfaction with the occurrence of adverse reactions or inadequate pain control (P>0.05). Conclusions Constructing the PCIA nursing management protocol based on guideline with nurses in the ward proposed as principal partners is suitable for routine operation. It can truly play the role of nurses in postoperative pain management, improve postoperative pain management quality and patients′ satisfaction, also provide patients with safe and effective analgesic service.
