理论培训结合安全核查表在麻醉危急事件模拟教学中的应用价值

摘要:

目的 探讨理论培训结合安全核查表模式对低年资住院医师处理麻醉危急事件及其在麻醉模拟教学中的应用价值。 方法 采用随机数字表法将27例我院低年资(1~3年)临床住院医师分为3组(每组9例):A组、B组与C组。A组接受理论授课与安全核查表相关培训,B组仅采用安全核查表,C组凭借临床经验处理麻醉危急事件,每个大组随机抽取3例组成a小组、b小组与c小组,在进入高保真模拟手术室后随机分配到8个麻醉危机事件场景中进行模拟演练,评估最佳实践得分率、关键步骤失败率,并采用问卷调查方式了解参与者后期安全核查表执行意向情况。 结果 与C组比较,A组和B组对危急事件处理的最佳实践得分率明显增高(P<0.05);与B组比较,A组最佳得分率增高(P<0.05)。与C组比较,A组和B组关键步骤的失败率明显降低(P<0.05)。90%以上的参与者表示安全核查表简单易用,愿意继续参加该种形式的麻醉模拟培训。 结论 危急事件理论培训与安全核查表的有效结合可明显提高住院医师处理麻醉危急事件的准确率和有效率,提高了麻醉模拟教学的积极性,对模拟教学具有重要的临床价值。

Abstract:

Objective To explore the application value of principle training and crisis checklist for low-grade residents to deal with anesthesia emergency and anesthesia simulation teaching. Methods Twenty-seven young clinical residents in low-grades (1-3 years) were randomly divided into 3 groups by random number table (n=9). Residents in group A received training of theory and crisis checklists. Residents in group B only received training of crisis checklists. Residents in group C relied on clinical experience to deal with anesthesia critical events. Each group of participants were randomly assigned to 8 anesthesia crisis events after entering the high-fidelity simulation operating room to perform simulation exercises. The best practice score rate and critical step failure rate were evaluated. Questionnaire survey was used to estimate the participants′ intention of using crisis checklist. Results The best practice scores of critical event treatment in group A and group B were significantly higher than score in group C (P<0.05). The best practice score for group A was higher (P<0.05) than the score in group B. The failure rates of critical steps in group A and group B were significantly lower (P<0.05). More than 90% of participants indicated that the crisis checklist was simple and easy to use. They were willing to continue to participate in anesthesia simulation training. Conclusions The theory training and crisis checklists can effectively improve the accuracy and efficiency of young residents in handling anesthesia emergencies, improve the enthusiasm of anesthesia simulation teaching and have important clinical value for simulation teaching.
