摘要：

为探讨心血管病患者行重大非心脏手术（major non-cardiac surgery, MNCS）围手术期处理的特殊性，总结近年来已有的和新进展的非心脏手术围手术期死亡的相关高危因素评估系统。包括目前使用最广泛的改良心脏风险指数（Revised Cardiac Risk Index， RCRI），Gupta′s Index及其发展出来的网上风险计算器，通用型围手术期风险评估计算器，还有如预测术后病死率的术前评估量表（Preoperative Score to Predict Postoperative Mortality, POSPOM）和非心脏手术中心血管事件发生率的队列研究（Vascular Events In Noncardiac Surgery Patients Cohort Evaluation, VISION）等研究所得出的初步结果等。在目前临床中心血管患者行MNCS的术前评估情况已有不少可观的进展，大范围的临床流行病学调查研究仍需要用以进一步的发展和完善这一评估体系。

Abstract:

In order to explore the perioperative management in patients with cardiovascular diseases undergoing major non-cardiac surgery(MNCS), this review summarizes the existing and newly developed evaluation index of perioperative high-risk factors related to non-cardiac surgery in recent years, such the generally Revised Cardiac Risk Index(RCRI), Gupta's Index and other calculator developed by studies and universal perioperative surgical risk calculator. Other studies like Preoperative Score to Predict Postoperative Mortality (POSPOM) and Vascular events In noncardiac Surgery Patients Cohort Evaluation (VISION) are also impressive. There have been considerable advances in the preoperative assessment for patients undergoing MNCS in clinic currently, extensive clinical epidemiological investigation is still needed to improve the evaluation index.
