麻醉深度指数和Narcotrend指数的临床应用比较

Comparison of clinical application of depth of anesthesia index and Narcotrend index during intravenous anesthesia

摘要:

目的:探讨麻醉深度指数(Depth of anesthesia index , AI)在全凭静脉麻醉中的应用,并和Narcotrend指数(Narcotrend index, NI)进行比较。方法:ASAI-II级,择期在全麻下行支撑喉手术的患者30例,年龄18-65岁,每例患者同时监测AI、NI以及丙泊酚效应室浓度(effect-site concentration , Ce)。麻醉诱导气管插管后,持续泵注丙泊酚、瑞芬太尼以维持合适麻醉,记录不同时间点AI、NI、Ce。结果:AI与NI、Ce的相关系数分别为0.913,-0.599,NI和Ce的相关系数为-0.584,AI与NI、Ce三者具有良好的相关性。结论:AI和NI均能反映患者不同刺激下的麻醉深度,AI用于全凭静脉麻醉可较好的控制麻醉深度,指导合理用药,避免患者术中知晓。

Abstract:

Objective :Discuss the appilication of depth of anesthesia index during total intravenous anesthesia, and compare with Narcotrend index . Methods :Choosing 30 cases of patients who were for Laryngeal surgery, aged 18-65, ASA I to II. All patients were monitored by AI,NI and propofol effect-site concentrations. After anesthetic induction and endotracheal intubation, Propofol and remifentanil were continuously infused to maintain adequate depth of anesthesia. Recorded AI, NI, and Ce at different time point. Results :The correlation coefficients of AI, NI and Ce are 0.913,-0.599, the correlation coefficients of NI and Ce are -0.584, there is a good correlation between AI、NI and Ce. Conclusions: Both of AI and NI can reflect the depth of anesthesia under different stimulation. During total intravenous anesthesia, AI can better control the depth of anesthesia, help guide rational drug use and avoid intraoperative awareness.
