含乳棒棒糖对健康志愿者胃排空的影响:一项前瞻性交叉设计对照研究

Effect of milky lollipop on gastric emptying in healthy volunteers: a prospective randomized crossover study

摘要:

目的:禁食水在降低反流误吸风险的同时也会增加患者的不适感,目前指南推荐在术前2h以上饮用清液体。棒棒糖有助于减轻饥饿感,还有利于改善焦虑,但含乳棒棒糖对患者胃排空的影响还有待明确,以确保其术前使用的安全性。 .方法:本研究为前瞻性交叉设计随机对照试验,共纳入20例健康志愿者,接受两阶段观察,同一患者在两阶段分别食用或不食用含乳棒棒糖。测定口服200mL清水后,食用或不食用含乳棒棒糖者胃容积的变化。通过超声测定胃窦面积来计算胃容积。主要研究指标为胃排空半衰期。次要研究指标为胃排空时间、干预后2h时残余胃容积。 结果: 两阶段受试者胃排空时间无显著差异,食用棒棒糖阶段胃排空半衰期为20(6)min,不食用棒棒糖阶段为23(9)min(P=0.33)。两阶段受试者干预后2h残余容积与基础值相比均无显著差异。 结论:含乳棒棒糖不影响健康受试者对清水的排空速度和饮水后2h患者胃容积。

Abstract:

Objective: Fasting is essential for lowering the risk of regurgitation and aspiration, while it may cause discomfort in the patients. Current guidelines recommend intake of clear fluid at 2h before induction. Lollipop helps to alleviate the feeling of hungry and anxiety. However, the effect of milky lollipop on gastric emptying is still unclear. Methods: This prospective randomized observer-blind crossover trial was performed on 20 healthy volunteers who attended two separate study sessions: Control and Chewing milky lollipop. Gastric volume was measured after intake of 200mL water. Serial measurements of the antral area were then performed by ultrasound and gastric fluid volume were calculated. The primary endpoint was half-time to gastric emptying. Secondary endpoints were total gastric emptying time and gastric fluid volume 2h later. Results: Gastric emptying of water was not different between sessions; the mean (SD) t1/2 was 23 (9) min in the Control session and 20 (6) min in the lollipop chewing session (P=0.33). There was no significant difference between sessions in gastric fluid volumes measured before ingestion of water and 120 min later. Conclusion: Milky lollipop does not affect gastric emptying of water, nor the gastric fluid volume measured 2h after ingestion of water.
