【摘要】 目的 探究猪经胸超声心动图(transthoracic echocardiography, TTE)监测模型的建立及常用切面的获取方法。 方法 挑选10只体格正常的健康小猪,12~18个月,体重45~60 kg。采用Philips CX‑50 超声系统和TTE探头进行TTE相关操作。在全身麻醉状态下,连续两次记录5个TTE常用切面[胸骨旁左室长轴切面(parasternal long‑axis view, PLAX)、胸骨旁短轴切面(parasternal short‑axis view, PSAX)、心尖部四腔心切面(apical 4‑chamber view, A4C)、剑突下四腔心切面(subcostal 4‑chamber view, S4C)、剑突下下腔静脉长轴切面(subcostal inferior vena cava long‑axis view, SILAX)]的获取情况,并用Kappa检验分析获取情况的一致性。 结果 PLAX两次获取率均为100%,提示两次切面获取一致性较好(Kappa=1);PSAX两次获取率均为90%,提示两次切面获取一致性较好(Kappa=1);A4C两次获取率分别为80%、70%,提示两次切面获取一致性一般(Kappa=0.61);S4C两次获取率均为90%,提示两次切面获取一致性较好(Kappa=1);SILAX两次获取率分别为80%、90%,提示两次切面获取一致性一般(Kappa=0.62)。 结论 将TTE探头置于猪相应体表标志处可以稳定获取TTE常用切面。

Abstract:

【Abstract】 Objective To explore the establishment of a pig monitoring model for transthoracic echocardiography (TTE) and the acquisition method of commonly used sections. Methods Ten physically normal pigs, weighing 45‒60 kg, aged 12‒18 months, were selected in the current study. The Philips CX‑50 ultrasonic system and TTE probe are used for TTE related operations. Five commonly used sections were recorded for consecutively twice consecutively under general anesthesia, including the parasternal long‑axis view (PLAX), the parasternal short‑axis view (PSAX), the apical 4‑chamber view (A4C), the subcostal 4‑chamber view (S4C) and the subcostal inferior vena cava long‑axis view (SILAX), and use Kappa test to analyze the consistency of the acquisition. Results The conditions of the five commonly used TTE sections were showed as follows: the two acquisition rates of the parasternal long axis view were both 100%, suggesting that the two acquisitions had a good consistency (Kappa=1). The two acquisition rates of the parasternal short‑axis view were both 90%, suggesting that the two acquisitions had a good consistency (Kappa=1).The two acquisition rates of the apical 4‑chamber view were 80% and 70% respectively, suggesting that the two acquisitions had a common consistency (Kappa=0.61). The two acquisition rates of the subcostal 4‑chamber view were both 90%, suggesting that the two acquisitions had a good consistency (Kappa=1). The two acquisition rates of the subcostal IVC long‑axis view were 80% and 90 respectively, suggesting that the two acquisitions had a common consistency (Kappa=0.62). Conclusion The operation method that the TTE probe is placed on the corresponding body surface marker of the pig can stably obtain the commonly used sections of TTE.
