摘要:

双腔支气管导管(double‑lumen endotracheal tube, DLT)是胸外科手术中最常使用的气道管理工具。人体气道的解剖特点为右主支气管较短,且右肺上叶开口可能会存在解剖变异,使用现有的右侧双腔支气管导管(right double‑lumen endotracheal tube, RDLT)可能会有隔离不良、容易移位、阻塞右肺上叶等风险。为了减少此类情况的发生,各种新型RDLT设计应运而生,为临床提供了一些潜在的新的解决办法。文章总结了目前比较有临床价值的RDLT改进方案,以期为有关临床问题和产品的研发提供新的解决思路。

Abstract:

Double‑lumen endotracheal tube (DLT) are the most commonly used airway management tool in thoracic surgery. Due to the anatomical characteristics of the human airway, the right bronchus is shorter than the left. Thus, the opening of the right upper lobe may present anatomical variations. Using the existing right double‑lumen endotracheal tube (RDLT) may lead to poor separation, displacement tendency, and obstruction of the right upper lobe. To avoid these complications, various new RDLT designs have emerged, offering some novel potential clinical solutions. This paper summarizes the current clinically relevant improvement solutions of the RDLT, which could provide new ideas for product development and solutions of the existing clinical problems.
