不同静脉PICC置管长度与身高相关性的实验研究和临床观察

【摘要】 目的:探讨经外周静脉中心静脉置管术(PICC)置管长度与患者身高的相关性,并进行线性回归分析,得出回归方程,评价其临床使用价值。方法:随机选择93例中国人体尸体标本,分别测定身高(H)、左贵要静脉(L1)、左头静脉(L2)、右贵要静脉(R1)、右头静脉(R2)的长度,分别将L1、L2、R1、R2和H进行相关性分析,并进行线性回归分析,得出回归方程;选择10例口腔颌面外科上颌骨切除手术患者,同时进行PICC和经右锁骨下静脉中心置管术,术中同时在不同的4个时点测量CVP,记录为CVP1、CVP2。结果:L1、L2、R1、R2和H的相关性分析均呈正相关;L1、L2、R1、R2和H相关系数分别为:0.683、0.698 0.702、0.673;L1和H的回归方程为:y=-2.390+0.294x ,(其中x为自变量取值,y为应变量取值,下同;)L2和H的回归方程为:y=-2.033+0.31x ;R1和H的回归方程为:y=-2.669+0.281x ;R2和H的回归方程为:y=-1.44+0.291x ;CVP1和CVP2比较,p >0.05。结论:PICC置管长度与患者身高存在正相关,其回归方程可用于指导临床,有一定的临床使用价值。

Abstract:

[Abstract] Objective To investigate the correlation between heigh and the length of different vien with PICC,and then evaluated the regression equation basic on the linear-regression analysis in clinic. Methods 93 adult cadavers of Chinese were chosed at randomly ,measuring respectively the height of the body (H)and the length of left basilic vein(L1)、left cephalic vein(L2) 、right basilic vein(R1) 、right caphalic vein (R2).then the correlativity and the regression analysis between the H and the L1、L2 、R1 、R2 were performed respectively,abtaining regression equation. 10 patients of maxillary bone surgery were chosed ,PICC and Center of the right subclavian vein catheterization were performed at the same patient , recording respectively the CVP1 and CVP2 at the same time on 4 different time positions. Results: L1、L2、R1、R2 and H correlation analysis showed a positive correlation,the correlation coefficients were :0.683、0.698 0.702、0.673;the regression equation between L1、L2、R1、R2 and H were : y=-2.390+0.294x;y=-2.033+0.31x;y=-2.669+0.281x;y=-1.44+0.291x ;(X values is independent avariable,Y values is dependent avariable); comparison between CVP1 and CVP2 , p >0.05。 Conclusion : The relativity between heigh and the length of different vien with PICC were positive correlation,the regression equation were applied in clinic.
