Abstract: Background Silent information regulator protein 1(Sirt1) is a NAD-depended class III histone/nonhistone deaceytalase, which regulates various biological processes. Not only does Sirt1 play an important role in anti-aging, it also protects cardiomyocytes from ischemia and reperfusion(I/R)injury. Objective To review the mechanisms of Sirt1’s protecting cardiomyocytes from myocardial I/R injury and several ways to activating sirt1. Content Sirt1 alleviates the I/R injury by increasing the generation of reactive oxygen species scavengers, inhibiting the apoptosis, promoting autophagic flux, attenuating the inflammatory reactions and promoting mitochondrial biogenesis. Activating Sirt1 is an important method in attenuating the I/R injury. Trend Sirt1 will be one of key targets in alleviating the I/R injury as the research about its protective mechanism is deepening.
