【摘要】 背景 近年来,肥胖人群不断增加,肥胖本身及其并发症使得肥胖患者经历手术的概率增加,其围术期处理对麻醉医师提出了挑战。短效、迅速的麻醉药物联合应用是全凭静脉麻醉的发展趋势,其中异丙酚和瑞芬太尼药物组合在短小手术中有其优越性。运用靶控输注系统以指定的靶浓度持续输注,能更准确地调节并稳定静脉药物血浆浓度。 目的 探讨异丙酚与瑞芬太尼靶控输注用于肥胖人群的可行性及前景。 内容 阐述肥胖对静脉麻醉药物使用的影响,根据近年发表文献分析异丙酚与瑞芬太尼靶控输注系统在肥胖人群中运用的可行性。 趋向 有待开发出更适合肥胖人群的药代动力学模型,从而更好地满足临床。

Abstract:

【Abstract】 Background Obese people, who is increasing in recent years, has higher probability of surgery because of obesity itself and/or its complications. Its perioperative management may be a major challenge for anesthesiologists. Combined use of rapid-onset, short-acting narcotic drugs is the development trend of total intravenous anesthesia. The co-administration of propofol and remifentanil has many advantages in short surgeries. Target Controlled Infusion(TCI) systems can administer drugs at a specific concentration, ensuring a more accurate and stable target concentration. Objective To investigate the feasibility and prospects of TCI with propofol and remifentanil in obese subjects. Content The effects of obesity on intravenous anesthetics were elaborated. The feasibility of TCI with propofol and remifentanil in obese subjects was analyzed according to the recently published literatures. Trend Pharmacodynamic models for TCI systems in obese patients are expected in clinic.
