Advancement about the effect of low-dose dopamine on renal function

摘要:

背景 以往报道,静脉剂量小于5 μg·kg-1·min-1的低剂量多巴胺(low-dose dopamine, LDD)可通过作用于肾脏多巴胺受体增加肾血流灌注和肾小球滤过率(glomerular filtration rate, GFR)而起到肾保护作用。然而近年的大量研究发现LDD无法彻底改善肾功能及患者远期生存率,LDD的应用质疑逐年增多。 目的 综述LDD的药理学效应及对肾功能影响的新进展,探讨其影响机制和影响因素,为LDD在临床中的应用提供参考。 内容 LDD由于对肾脏的特殊作用,曾一度广泛用于临床,但其影响肾功能的机制较复杂,可能会导致肾脏需氧量上升、管球反馈破坏,且其发挥作用也受多种因素影响,无法保证得到预想的肾保护效果。 趋向 LDD作为一种传统的肾功能保护药目前尚不推荐常规使用,其远期应用前景有待重新考量。

Abstract:

Background It has been reported that low-dose dopamine(LDD)(intravenous dosage less than 5 μg·kg-1·min-1)can protect the kidney against hypoperfusion by increasing renal blood flow and glomerular filtration rate(GFR). However, in recent years, novel studies have demonstrated that LDD is not able to ameliorate the renal function and increase the long term survival rate of patients at risk of kidney injury essentially. Therefore, LDD suffered queries from more and more scholars, as a result, the application of LDD has become a controversial issue. Objective To summarize the advancement about pharmacological effects of LDD on the renal function as well as its influential factors and underlying mechanisms. Meanwhile, to provide practical suggestions for the clinical application of LDD. Content Due to its specific effect on kidney, LDD was once widely used clinicaly in the past, however, its effect is influenced by a variety of factors and cannot be guaranteed. Meanwhile, the mechanism of it's action on the kidney is also complicated, which means that LDD may lead to increasing the kidney oxygen demand and depressing the tubuloglomerular feedback system responsiveness. Trend LDD, as a conventional drug for kidney protection, is not yet to be recommended for routine use at present and its application prospect should be reconsidered.
