摘要:

背景 呼气末二氧化碳分压(end tidal carbon dioxide tension, PETCO2)监测具有无创、简便、快捷、直观等优点,被广泛用于监测通气功能。同时PETCO2与肺血流灌注状况有密切关联,间接地同心排血量相关,故其还可作为心功能和循环动力的监测及评估指标。一直以来认为PETCO2与动脉血二氧化碳分压(arterial carbon dioxide tension, PaCO2)之间有很好的相关性。 目的 总结可能影响PETCO2与PaCO2相关性的有关因素,探讨可能存在的病理生理机制。 内容 结合近年来的相关文献,通过手术、患者基本情况及麻醉等三方面,就对PETCO2与PaCO2相关性产生影响的有关因素作一总结和回顾。 趋向 PETCO2与PaCO2在一般情况下相关性良好,也有相应的生理机制证明。但存在混杂较为复杂的因素时,两者的相关性还需要深入研究和不断探索。

Abstract:

Background End tidal carbon dioxide tension (PETCO2) monitoring has a lot of advantages like noninvasive, convenient, fast and intuitive, and it is widely used for monitoring the ventilation function. At the same time, PETCO2 is also associated with pulmonary perfusion well, so it can be used as a monitoring and evaluation index of heart function and circulation dynamics indirectly. That has been a good correlation between PETCO2 and arterial carbon dioxide tension(PaCO2). Objective To summarize the factors may influence the correlation between PETCO2 and PaCO2, and to explore the pathophysiological mechanism may exist. Content We performed a review of the factors that affect the relativity between PETCO2 and PaCO2 from the operation's type, the basic condition of the patients and the aspect of anesthesia. Trend In general, there is a good correlation between PETCO2 and PaCO2 in normal condition, which has also been proved in corresponding physiological mechanism. However, the correlation between them still needs further research and exploration in a mixed more complex condition.
