One patient for acute cerbral infarction of a large area， with ASA grade Ⅳ， scheduled for elective surgeries combing adical gastrectomy for gastric antrum cancer and adrenalectomy under general anesthesia. The anesthesia management is complicated for non-cerebral vascular operation of the patients for acute cerbral infarction， more attention should be paid during the perioperative period.
