摘要:

背景 机械通气是临床上最常用的呼吸支持手段,但长时间机械通气可导致膈肌萎缩和收缩功能障碍,即呼吸机相关膈肌功能障碍(ventilator-induced diaphragm dysfunction, VIDD),并与重症患者的脱机困难密切相关。VIDD发病机制复杂,临床上仍缺乏有效防治策略。 目的 综述VIDD的主要相关发生机制,特别是早期启动和调节机制,以及防治措施,寻找潜在的深入研究的突破点。 内容 阐述VIDD的研究概况、病理生理变化、发生机制和防治措施。 趋向 VIDD的确切机制和有效防治措施仍需进一步研究阐明。

Abstract:

Background Mechanical ventilation is one of the most common clinical respiratory support strategies. However,prolonged mechanical ventilation can cause diaphragmatic atrophy and contraction dysfunction, namely ventilaor-induced diaphragmatic dysfunction(VIDD), which is related to weaning failure of critically ill patients. Objective The pathogenesis of VIDD is complex and it is still lack of effective prevention and control strategies in clinical treatment. The current article reviewed the mainly related mechanisms, especially the early starting and regulating mechanisms as well as the prevention and treatment measures of VIDD, looking for potential research breakthrough. Content This article described research backgrounds, pathophysiological alterations, mechanisms and prevention measures of VIDD. Trend The precise mechanisms and effective prevention measures of VIDD still need further researches to clarify.
