心脏外科手术中应用自体血回收技术对血液保护效果的评价-Meta分析

【摘要】 目的 试图评价心脏外科手术中应用血液回收技术对血液保护的效果。方法 计算机检索Cochrane图书馆、Pubmed、EMbase、CINAHL等中外生物医学数据库。收集关于心脏外科手术使用自体血回收的随机对照试验,按Cochrane系统评价方法,评价所纳入研究的文献质量,并提取有效数据后采用RevMan5.1软件进行Meta分析。结果 纳入11项研究,共计2046名患者。Meta分析结果显示:结果显示应用血液回收设备能够减少围术期红细胞的使用率。[RR=0.68,95% CI (0.53, 0.87)]. 应用血液回收设备能够减少围术期新鲜冰冻血浆的使用率。[RR=1.53,95% CI (1.02, 2.31)]。血液回收设备组与对照组相比围术期血小板的使用率无统计学差异。[RR=0.86,95% CI (0.56, 1.32)],应用血液回收设备组与对照组相比术后并发症的发生率无统计学差异。[RR=1.14,95% CI(0.92, 1.41)]。结论 目前的证据表明心脏手术中使用自体血回收装置可以减少红细胞和新鲜冰冻血浆的输注,具有一定的血液保护的效果。此外,自体血回收并不会增加术后并发症如术后出血、感染、肾衰、认知功能障碍的发生率。

Abstract:

【Abstract】Objective Effects of autologous blood withdrawal-reinfusion on perioperative Blood protection in patients undergoing cardiac surgery.Methods We electronically searched Cochrane library,Pubmed,EMbase,CINAHL,Web of Science,CBM,CNKI.Randomized controlled trials (RCTs) were indentified about autologous blood withdrawal-reinfusion on perioperative Blood protection. Study selection and meta-analysis were conducted which according to the Cochrane Handbook for systematic reviews. Date were extracted from these trials by 3 reviewers independently and analyzed by RevMan5.1 software. Results The results showed that the application of blood collection equipment can reduce the use of red blood cells in the perioperative period. CI [RR=0.68,95% (0.53, 0.87). The use of blood recycling equipment to reduce the use of fresh frozen plasma. CI [RR=1.53,95% (1.02, 2.31)]. There was no significant difference in the use of the blood group in the treatment group compared with the treatment group. CI [RR=0.86,95% (0.56, 1.32)], there was no difference in the incidence of postoperative complications in the patients with the use of blood recovery equipment. CI [RR=1.14,95% (0.92, 1.41)]. Conclusions the current evidence suggests that the use of autologous blood recovery device in cardiac surgery can reduce the transfusion of allogeneic blood products or red blood cells, which has a very good effect. In addition, autologous blood collection does not increase the incidence of postoperative complications such as bleeding, infection, renal failure and cognitive dysfunction.
