椎管内麻醉期间行音乐干预对急诊剖宫产产妇的影响

The influence of using music during spinal-epidural anesthesia on parturient in emergency cesarean section

摘要:

目的 观察并比较在椎管内麻醉期间行背景音乐干预对急诊剖宫产产妇焦虑、疼痛、满意度的影响。 方法 选取急诊剖宫产产妇110例纳入观察。采用随机数字表法将产妇分为音乐组和对照组,每组55例。音乐组产妇在进入手术室前选择自己平时最喜欢的音乐,用手机连接外放音响在手术间播放,对照组则不听音乐。两组均行标准的蛛网膜下腔-硬膜外联合麻醉操作,并将麻醉平面控制在T6水平。观察并比较两组产妇的焦虑水平、疼痛程度及满意度。 结果 最终99例产妇纳入统计分析,音乐组50例,对照组49例。音乐组产妇麻醉生效后即刻的数字评分法(Numeric Rating Scale, NRS)和状态焦虑表(State Anxiety Inventory, S-AI)评分较对照组低(P<0.05),两组VAS评分差异无统计学意义(P>0.05)。音乐组产妇整个麻醉过程的满意度比对照组高(P<0.05);并且音乐组产妇对于以后若再次行剖宫产手术,在椎管内麻醉期间想要听音乐的需求明显高于对照组(P<0.05)。 结论 在椎管内麻醉期间行背景音乐干预可一定程度降低急诊剖宫产产妇的焦虑程度,并且可以提升产妇对麻醉的满意度。

Abstract:

Objective To observe and compare influences of music use during spinal-epidural anesthesia on parturient anxiety, pain and satisfaction in emergency cesarean section. Methods One hundred and ten parturients for emergency cesarean section were divided randomly into two groups, with 55 parturients each. The music group listened to the patient′s preferred music on a mobile phone through an external amplified speaker, the control group did not listen music. All women received a standardized spinal-epidural anesthesia. The outcomes included obstetric parameters, hemodynamic parameters, anxiety, pain and patient satisfaction. Results A total of 99 parturients were included in the statistical analysis. The music group experienced low anxiety as measured by Numeric Rating Scale (NRS) and State Anxiety Inventory (S-AI) scores immediately after spinal-epidural anesthesia (P<0.05). However, no significant difference in mean pain scores in two groups (P>0.05). A higher patient satisfaction rate was observed in the music group during the whole process of anesthesia (P<0.05). The desire for music use for future spinal-epidural anesthesia was higher in the music group (P<0.05). Conclusions Music use during spinal-epidural anesthesia in emergency cesarean section promotes lower postprocedure anxiety and provides a higher patient satisfaction for emergency cesarean section.
