围手术期患者容量状态评估及液体管理研究进展

摘要:

合理的围手术期液体管理是影响患者快速康复的关键因素之一。优化液体治疗能够提高组织灌注和氧供,有助于改善微循环,进而降低患者的病死率和术后住院时间。对患者容量状态的准确评估是围手术期管理的基础。文章对围手术期患者容量状态评估方法进行综述,内容包括静态指标、动态指标以及经胸心脏超声和肺超声下的各种参数等,为麻醉医师围手术期液体管理提供新的思路。

Abstract:

Rational perioperative fluid management is one of the key determinants responsible for enhanced recovery of patients after surgery. Hypovolemic condition or hypervolemia is closely associated with an increased postoperative mortality or morbidity. Optimal fluid administration facilitates the improvement of tissue perfusion and oxygen supply, so as to avoid microcirculation dysfunction, unnecessary fluid loading, and reduce patient mortality and hospitalization stay. Therefore, accurate assessment of volume status is one of important issues in perioperative management for these patients. In the paper, diverse methods of assessing perioperative volume status of patients including static and dynamic parameters as well as noninvasive transthoracic ultrasound are critically reviewed. The advancements in assessment of the volume status of patients and perioperative fluid management are reviewed to provide new thoughts in fluid therapy of the surgical patients in this article.
