右美托咪定在功能神经外科的围手术期应用进展

Application of dexmedetomidine in functional neurosurgery during the perioperative period

摘要:

右美托咪定(dexmedetomidine, Dex)是一种α2肾上腺素能受体激动剂,其镇静过程接近于自然睡眠,易于唤醒,同时具有镇痛、抑制交感神经活性、无明显呼吸抑制等特性。功能神经外科手术中进行的电生理功能监测对于定位手术区域和决定手术能否成功至关重要。目前证据表明低剂量Dex不干扰电生理监测,适用于神经外科唤醒麻醉手术区域定位,以及脑深部电刺激(deep brain stimulation, DBS)术中进行微电极记录(microelectrode recording, MER)等。文章总结了功能神经外科和麻醉管理要点、Dex的作用机制、Dex应用于功能神经外科的优势及其相关临床研究进展,以期合理应用Dex,优化麻醉管理和改善患者预后。

Abstract:

Dexmedetomidine (Dex), an α2 adrenergic agonist, provides sedation that resembles natural sleep, as well as analgesia and sympathetic neuronal activity inhibition without respiratory depression. Besides, patients are readily be arose. Monitoring of electrophysiological function of the brain during the functional neurosurgical procedure is crucial for localization of the operational brain region and the results of operation. Current reports indicate that a low dose of Dex does not interfere with electrophysiologic monitoring, which allows brain mapping during awake craniotomy and microelectrode recording (MER) during deep brain stimulation (DBS) surgery. This review summarizes the category of functional neurosurgery and key points of anesthetic management, the pharmacological properties of Dex, the advantages of Dex in functional neurosurgery, and related clinical research in recent studies to rationally use Dex to optimize anesthetic management and improve patients' prognosis.
