自体血小板分离回输对急性A型主动脉夹层患者 P⁃选择素和炎性因子的影响

Effect of autologous plateletpheresis and back‑transfusion on P‑selectin and inflammatory cytokines in patients with acute type A aortic dissection

摘要:

目的评价自体血小板分离回输对急性A型主动脉夹层(acute type A aortic dissection, ATAAD)患者P⁃选择素和 炎性因子水平的影响,以探讨其肺保护作用。方法选择拟在CPB和深低温停循环下行主动脉弓人工血管替换和象鼻支架 植入术的ATAAD患者48例,年龄24~68岁,采用随机数字表法将患者分为两组(每组24例):对照组(C组)和血小板分离组(P 组)。P组患者于全身麻醉后进行急性血小板分离,采集富血小板血浆并于CPB结束后回输;C组输入等量6%羟乙基淀粉130/ 0.4。所有患者分别于麻醉诱导前(T1)、CPB开始前(T2)、CPB结束后30 min(T3)和术后6 h(T4)抽取动脉血行血气分析,计算 PaO2/FiO2和肺泡⁃动脉氧分压差(alveolar⁃arterial oxygen partial pressure difference,P(A⁃a)O2),同时抽取静脉血检测血浆P⁃选择素 和炎性因子(IL⁃6、IL⁃8、TNF⁃α)浓度、血小板计数及白细胞计数。结果与T1时比较:P组T2时血浆P⁃选择素、血小板计数、 白细胞计数均减少,P(A⁃a)O2升高(P<0.05);T3时PaO2/FiO2、血小板计数、白细胞计数减少,P(A⁃a)O2、P⁃选择素、IL⁃6、IL⁃8和TNF⁃α 升高(P<0.05);T4时PaO2/FiO2、血小板计数减少,P(A⁃a)O2、P⁃选择素、IL⁃6、IL⁃8和TNF⁃α升高(P<0.05)。与T1时比较,C组T2~T4 时PaO2/FiO2、血小板计数减少,P(A⁃a)O2、P⁃选择素、IL⁃6、IL⁃8和TNF⁃α升高,T4时白细胞计数升高(P<0.05)。与C组比较,P组 T2~T4时PaO2/FiO2升高,P(A⁃a)O2、P⁃选择素、IL⁃6、IL⁃8、TNF⁃α、白细胞计数降低,T2、T3时血小板计数减少(P<0.05)。结论自 体血小板分离回输可降低ATAAD患者术中P⁃选择素及IL⁃6、IL⁃8、TNF⁃α水平,减轻炎症反应,具有肺保护作用。

Abstract:

Objective To evaluate the effect of autologous plateletpheresis and back⁃transfusion on P⁃selectin and inflamma⁃ tory cytokines in patients with acute type A aortic dissection (ATAAD) to explore its lung protection. Methods Forty⁃eight consecu⁃ tive type⁃A aortic dissections patients undergoing aortic arch replacement and elephant trunk stent implantation under cardiopulmonary bypass (CPB) and deep hypothermic circulatory arrest, aged from 24 to 68 years old, were divided into two groups (n=24): control group (group C) and plateletpheresis group (group P), according to the random number table method. In group P, autologous plateletpheresis was performed after general anesthesia, and autologous platelet⁃rich plasma (aPRP) was back⁃transfused after CPB. In group C, the pa⁃ tients were infused with normovolemic 6% hydroxyethyl starch(130/0.4). In all patients, the arterial blood was drawn for blood gas anal⁃ ysis, arterial partial pressure of oxygen (PaO2)/fraction of inspiratory oxygen (FiO2) and alveolar⁃arterial oxygen partial pressure differ⁃ ence (P(A⁃a)O2 ) were calculated, and the venous blood was drawn for measuring the concentration of P⁃selectin and inflammatory cyto⁃ kines [interleukin (IL)⁃6, IL⁃8, tumor necrosis factor⁃α (TNF⁃α)], platelet counts and white blood cell counts before anesthesia induc⁃ tion (T1), before CPB (T2), at 30 min after the end of CPB (T3) and at 6 h after operation (T4). Results Compared with those values at T1: P⁃selectin, platelet count, and white blood cell count decreased, while P(A⁃a)O2 increased at T2 in group P (P<0.05). Compared with those at T1, PaO2/FiO2, platelet count and white blood cell count decreased, while P(A⁃a)O2, P⁃selectin, IL⁃6, IL⁃8, and TNF⁃α increased at T3 in group P (P<0.05). Compared with those at T1, PaO2/FiO2 and platelet count decreased, while P(A⁃a)O2, P⁃selectin, IL⁃6, IL⁃8 and TNF⁃α increased at T4 in group P (P<0.05). Compared with those values at T1, PaO2/FiO2, platelet count, P(A⁃a)O2, P⁃selectin, IL⁃6, IL⁃8 and TNF⁃α expression decreased from T2 to T4 time point , while white blood cell count increased at T4 in group C(P<0.05). Compared with group C, PaO2/FiO2 value increased, while P(A⁃a)O2, P⁃selectin, IL⁃6, IL⁃8, TNF⁃α values and white blood cell count decreased from T2 to T4, and platelet count decreased at T2 and T3 in group P (P<0.05) Conclusions Autologous plateletpheresis and back⁃transfu⁃ sion can reduce the concentrations of intraoperative and postoperative P⁃selectin and inflammatory cytokines (IL⁃6, IL⁃8, TNF⁃α) in pa⁃ tients with ATAAD. Thus, it has lung protection effect by reducing the inflammatory reaction.
