围术期使用普瑞巴林对非小细胞肺癌患者术后情绪及睡眠的影响

摘要:

目的 探讨围术期使用普瑞巴林对非小细胞肺癌患者术后情绪及睡眠的影响。方法 选取2022年9月至2022年10月在徐州医科大学附属医院接受胸腔镜下肺切除手术患者60例,采用随机数字表法将其分成观察组和对照组,每组30例。两组患者术后均采用静脉自控镇痛,对照组患者口服安慰剂,观察组患者于术前1 h~2 h口服普瑞巴林150 mg,术后1天~5天口服普瑞巴林75 mg,每日2次。术后采用医院焦虑抑郁量表(Hospital Anxiety and Depression Scale, HADS)、阿森斯量表评分(Athens Insomnia Scale, AIS)和匹兹堡睡眠质量指数量表(Pittsburgh Sleep Quality Index, PSQI)评价两组情绪状态及睡眠质量。同时记录两组患者的视觉疼痛模拟评分(Visual Analogue Scale, VAS)、围术期疲劳评测量表(Identity-Consequence Fatigue Scale, ICFS)及治疗期间不良反应发生情况。结果 观察组术后第1、2、3天焦虑、抑郁发生率和AIS评分均低于对照组,对比有统计学差异(均P0.05)。两组术后第7、30天焦虑、抑郁发生率和睡眠质量评分对比无统计学差异(均P0.05)。观察组术后镇痛满意度优于对照组(P0.05)。观察组术后ICFS评分均低于对照组,差异具有统计学意义(P0.05)。两组患者术后轻微不良反应发生率组间差异无统计学差异(P 0.05)。结论 围术期使用普瑞巴林可以缓解非小细胞肺癌患者术后焦虑、抑郁的情绪,改善术后睡眠质量,减轻术后疼痛和疲劳程度,且安全性较好。

Abstract:

Objective To investigate the effect of pregabalin on postoperative mood and sleep of patients with non-small cell lung cancer. Methods Sixty eligible patients undergoing thoracoscopic pneumonectomy in the Affiliated Hospital of Xuzhou Medical University from September 2022 to October 2022 were randomly divided into the observation group and the control group, with 30 cases in each group. Patient-controlled intravenous analgesia was used in both groups after the operation. Patients in the control group took placebo orally, while patients in the observation group took pregabalin 150 mg 1 hour~2 hours before operation, and then 75 mg twice a day, 1 day~5 days after the operation. Hospital Anxiety and Depression Scale (HADS), Athens Insomnia Scale (AIS) and Pittsburgh Sleep Quality Index (PSQI) were used to evaluate the emotional state and sleep quality of the two groups. At the same time, the Visual Analogue Scale (VAS) scores, the Identity-Consequence Fatigue Scale (ICFS) scores and incidence of adverse reactions during treatment of the two groups were recorded. Results The incidence rate of anxiety and depression, and the AIS scores of the observation group were lower than those of the control group on days 1, 2 and 3 after the operation, the differences were statistically significant (all P0.05). There were no significant differences in the incidence rate of anxiety and depression, and the scores of sleep quality between the two groups on days 7 and 30 after the operation (all P0.05). The analgesic satisfaction of the observation group was better than that of control group (P0.05). The ICFS scores of the observation group after the operation were lower than those of the control group, the differences were statistically significant (P0.05). The minor side effects showed no difference across cases (P0.05). Conclusion Perioperative use of pregabalin can relieve postoperative anxiety and depression in patients with non-small cell lung cancer, improve postoperative sleep quality, and reduce postoperative pain and fatigue, with good safety.
